I think it might be best if you and your family went to anutritionist. While the focus might be finding a meal plan for you, your family can be there to support you and they can see how they can support you.
It isn't mean when you refuse the food. You might want to say something before they start cooking such as, “Don’t worry about making me any, I'm going to stick to my meal plan."
As for buying healthy things, try to make a trip to the store with them. Also, you might want to try and be more specific than healthy things. 'Healthy things' varies from person to person. I think healthy things are fruits, veggies, water, and whole wheat bread. My mom thinks healthy things are completely different.
As for exercising, I found it awkward too. Check around town and see if you can find (I'm really sorry, but I'm assuming your female ) an all female gym, such as lady fitness. Everyone there is a woman so I felt more comfortable and less judged.

I hope that you make and reach a healthy goal.
