I know you said you don't want to bug your guidance counselor anymore, but it may be your best option. You do have documented conditions and you are seeing a psychologist. They may be able to develop some sort of action plan with you to help make sure you do as well as you can in your classes. You can't get out of every class or anything, but it may be nice to have a plan put in place that the teachers and everyone can follow to make sure you are getting the most out of your learning experience. They may even be more willing to work with you one-on-one, either before or after class, if they know what's happening. It's your guidance counselor's job to bother people, after all. Or you can talk to the school nurse or an administrator like a vice principal or principal and see what they say, but the school may e willing to work with you on some things.
I think you should also talk to your psychologist about the problems you are having with school, because they should have some suggestions for you.
