TeenHelp - New diagnosis and new medication?

Well I had my psychiatrist appointment today and after talking to her for a bit she decided to change my diagnosis from bipolar disorder to schizoaffective disorder. She also added an antipsychotic to my medication. On top of all that, she said if this medication doesn't help me she'll be sending me to a "severe chronic mental health clinic".
Does anybody know anything about this disorder? Or what I might be able to expect from a clinic like that?

I've also been thinking of applying for disability, so if anybody knows anything about that I would appreciate some tips or information.
